Completing N244

I need to complete a N244 application, but a bit stuck on sections 5, 8 & 9.

Im a LIP and have answered Q5 - at a telephone hearing, is that ok?

Section 8 asks what level of Judge - dont know what to put here, as this application relates to a RCJ permission to appeal hearing.

Section 9 asks who should be served - There are 5 Defendants and I was going to just put - all defendants/respondents, is that ok?

Also - how many copies should be sent to the Court, would it be - one for the court, one for the applicant, and one for each defendant?

It is unlikely that you will have a telephone hearing..but you can ask the court will ultimately make the decision for you.

you should name the parties to be served, either in full or as 1st 2nd 3rd defendant etc and provide enough copies for you ,the court and each other party.

Ive got to get this spot on, cant afford to fail on procedural matters.

A couple more queries - The 2-5th Defendants are all connected and are collectively represented by the same legal team. Should it be their legal representative who is shown at section 9, and if so would only one copy of the application and evidence be required to be served on the 2-5th respondents representative?

I have prepared a witness statement together with 4 exhibits as evidence in support of my application. There are only about 15 pages in total, wonder if these can be stapled and attached by paperclip to the application, or would the court expect an indexed bundle.
